Practical examples
It is the handover between teams that causes most of the delay. The individual tasks are usually completed within the expected time. What remains unclear is who should confirm that the next team has received the files. It was this gap in responsibility that several colleagues mentioned during the review. Adding more detail to the task instructions would not necessarily address it. What we need first is a clear point of contact for each handover.
